South Country Human Service Zone supports the elderly and/or disabled community that wants to continue to be successful in their own home environment. As a Direct Care Associate, you will connect with citizens of the State of ND to complete daily living activities with the goal of increasing their ability to be independent.Direct Care Associates will provide direct service to the elderly and/or disabled individuals in their home to enable them to continue to reside in their home/community. In this role, you will be responsible for providing environmental services and personal cares to eligible individuals, such as: laundry, shopping, housekeeping, meal preparation, transportation, bathing, hair care, etc.You will need to have attention to detail in this role because you will be responsible for accurately documenting activities through electronic visit verification and/or paper logs.This position requires daily travel to client homes throughout Dickey, LaMoure, and McIntosh Counties. Individual would participate in staffing and required training to maintain Qualified Service Provider status.Applicant should have strong verbal and written communication skills, computer literacy, collaboration, and decision-making skills. Minimum Qualifications

Must have a high school diploma or GED and two (2) years of work experience providing direct care to special populations. Active CNA license preferred.Would consider Direct Care Associate underfill, which requires a high school diploma or GED and one (1) year of work experience providing direct care to special populations. Active CNA license preferred.Selected candidate must successfully complete a standard background check. Must also have a valid driver's license with clean driving record, a reliable vehicle for transportation, and adequate auto insurance coverage.
