Career Description
Receive reports of alleged abuse or neglect of a vulnerable adult; complete an assessment for each adult, generally through a home visit; consult and coordinate with collateral entities; complete documentation of work done on case; arrange for, coordinate and collaborate with other agencies for provision of essential services; provide case management and counseling for the purposes of planning and providing social and other services needed by vulnerable adults; monitor the delivery of services through scheduled follow-up; conduct training and community outreach; and compile and maintain case files. This position will also participate in applicable court hearings.
Minimum Qualifications
Bachelor's degree in Social Work, Gerontology, Criminal justice, Nursing, Business administration, Counseling (including addiction counseling), Psychology, Occupational therapy, Physical therapy, Public health, or Sociology plus at least two (2) years of professional human services work experience as a social worker or human relations counselor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector.
